About the Position

We are seeking a full-time Adult Primary Care Physician to provide comprehensive medical care to members enrolled in an Institutional Special Needs Plan (ISNP). This role offers the opportunity to make a meaningful impact on medically complex seniors through proactive care management, interdisciplinary collaboration, and a strong focus on improving outcomes and preventing avoidable hospitalizations. J oin a growing, patient-centered healthcare organization serving Medicare members with complex chronic conditions in skilled nursing facilities, long-term care facilities, and other institutional settings throughout the Las Vegas area.

• Base Salary: $211,369–$317,053

Qualifications

• MD or DO degree from an accredited medical school.
• Completion of an accredited Family Medicine, Internal Medicine, or Geriatrics residency program.
• Board Certified or Board Eligible.
• Active Nevada medical license, or ability to obtain licensure.
• Valid DEA registration.
• Two or more years of experience in SNF, LTC, post-acute care, or complex senior care preferred.
• Experience with HCC coding, Medicare Advantage, HEDIS, and STARs documentation preferred.
• Comfortable practicing in institutional and post-acute care settings.
• Ability and willingness to travel between healthcare facilities in Nevada.
