Community Bridges is seeking one (1) part-time Medical Direct Support Professional (DSP) to join our team in Northfield, NH .

We proudly support more than 1,100 individuals and their families throughout the Greater Capital Area by identifying barriers to access and developing creative, person-centered solutions that foster meaningful community engagement.

The Medical Direct Support Professional (DSP) serves as a community connector , supporting individuals with medical and developmental needs to build meaningful relationships and inclusive community experiences. This role focuses on community mapping, promoting social inclusion, and supporting individuals in pursuing their interests, strengths, and goals-always emphasizing abilities over disabilities.

• Schedule - staff will be working 3 days per week with each shift being 12 hours in duration; staff will be considered full-time

• Education: High School Diploma or GED required

• Experience: Experience in human services or working with individuals with disabilities preferred

• Requirements:

  • Reliable transportation

  • Valid driver's license

  • Proof of automobile insurance

• $1,000 Sign-On Bonus

  • $500 paid at hire

  • $500 paid after 6 months of employment

  • Bonus available to new hires only; rehires and transfers are not eligible at this time

• Paid Training: Employees are paid their regular rate while attending required training

• Mileage Reimbursement: Approved mileage driven while on the clock is reimbursed

Less Than Part-Time:

• Mondays & Thursdays

• 7:00 a.m. – 3:00 p.m.

• Saturdays: 7-3pm & 3-11pm

• Assist individuals with planned activities and community involvement

• Support medical needs including:

  • G-tubes

  • Catheters

  • Medication administration

  • Oxygen use

  • Aspiration precautions and other medical supports

• Assess pain and discomfort in non-verbal individuals

• Develop relationship maps to identify social inclusion opportunities

• Support individuals in building and maintaining social networks

• Promote self-esteem through active community participation

• Provide individualized supports including personal care and skill development

• Assist with hygiene and personal care (bathroom use, bathing, dental hygiene, etc.)

• Participate in Person-Centered Planning and implementation of goals

• Complete required documentation, data collection, and monthly progress notes

• Assist with training and orientation of new staff as needed

• Distribute medication in accordance with He-M 1201

• Report all safety, health, or human rights concerns immediately

• Ensure compliance with state regulations

• Perform other duties as assigned
