We are seeking an experienced Discharge Planning Coordinator (LPN) to oversee the discharge process for our Sub‑Acute Unit and help residents transition safely and successfully to the next level of care. Responsibilities Coordinate and oversee resident discharge planning for the Sub‑Acute Unit Participate in utilization review and insurance/Medicare reviews Issue Medicare cut letters as needed Collaborate with interdisciplinary teams on treatment and discharge plans Provide social service support to residents and families Coordinate community resources and post‑discharge services Maintain accurate records and documentation Qualifications Master’s Degree in Social Work OR Active NJ LPN or RN license Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning Long‑term care or rehab experience preferred What We Offer Flexible scheduling opportunities Outstanding county benefits package Supportive team environment with growth opportunities Roosevelt Care Center at Old Bridge is proud to be an Equal Opportunity Employer. #J-18808-Ljbffr Roosevelt Care Center at Old Bridge
