Responsibilities
The Social Services Supervisor provides leadership, oversight, and advanced guidance for the delivery of psychosocial services within the interdisciplinary team (IDT). The Supervisor ensures that Master’s‑level Social Workers (MSWs) complete all regulatory assessments, care plans, and documentation in accordance with PACE, state, and internal InnovAge standard requirements, and that delegated tasks completed by Social Worker Assistants (SWAs) meet the same quality and compliance expectations.
This role supports participant safety, aging in place, and social determinants of health (SDOH) interventions; oversees grievances, service delivery requests, and incident investigations; guides staff through complex cases including housing instability, facility placement, eligibility barriers, and transitions of care; and serves as a key liaison among the IDT, Center Triad (Center Medical Director, Center Director, and Director of Nursing), and community partners. This role works in collaboration with behavioral health and other clinical leaders for clinical oversight, workflows, process and policy review. The Supervisor maintains a caseload proportional to census and staffing as such average time spent in each work function is relative to census and staffing agreed to by leadership.
Essential Functions and Work Responsibilities
Leadership, Supervision, and Clinical Oversight –
- Provide day‑to‑day supervision, coaching, and performance management for MSWs and SWAs.
- Ensure clinical quality, timely documentation, and adherence to all regulatory requirements.
- Review assessments, care plans, SDRs, grievances, incident documentation, and related work on a cadenced basis.
- Monitor caseloads, productivity, KPIs, and workload distribution while maintaining appropriate coverage plans.
- Oversee onboarding, training, competency development, and ongoing professional growth of social services staff.
- Carry out leadership responsibilities in accordance with organizational policies and applicable laws, including interviewing, training, assigning work, performance review, coaching, recognition, and problem resolution.
- Set department tone to support morale, collaboration, and positive employment culture; conduct one-on-one and team meetings to foster communication and alignment.
Regulatory, Process, and Quality Compliance –
- Ensure timely completion of all required MSW PACE specific psychosocial assessments and care plans.
- Oversee grievance, service determination request, appeal processes, and RCA documentation.
- Validate compliance with CMS, state, and InnovAge policies, standard operating procedures.
- Lead or support quality improvement initiatives, audits, and case reviews.
Participant Care Coordination and Complex Case Consultation –
- Provide advanced consultation on high‑risk cases, complex family dynamics, behavioral health concerns, substance use, and housing crises.
- Support escalation of safety concerns including abuse, neglect, and mandated reporting.
- Facilitate interdisciplinary collaboration with medical, behavioral health, eligibility, and operational teams.
- Lead or participate in care conferences, family meetings, and transition planning for complex cases.
Health, Safety, and Housing Oversight –
- Oversee all housing‑related processes including facility placements, level‑of‑care changes, tenancy disruptions, rehousing needs, and safety‑related interventions.
- Maintain relationships and coordinate with facilities, APS, ombudsman offices, and community organizations.
- Guide staff on connecting participants with SDOH resources, benefits, and navigation supports.
Communication, Documentation, and Coordination –
- Serve as a led communication hub between social work, caregivers, IDT members, Center Triad and external partners for escalated issues.
- Ensure timely, accurate documentation in EMRs and required systems.
- Maintain confidentiality and adherence to organizational policies.
Administrative Leadership –
- Assist with implementation of policies and procedures for the Social Work team in collaboration with leadership.
- Delegate logistical work to SWAs to support MSWs, comfort care coordination, and behavioral health.
- Partner with State Director and VP BH to implement best practices and standardized processes.
- Ensure adherence to agency and state policies through case record review.
- Monitor and evaluate service provision within the assigned catchment.
- Interview participants to address special concerns, escalating needs appropriately.
- Attend committees/meetings including Social Work Cross‑Site as identified.
Key Performance Indicators
The Social Services Supervisor is accountable for both team and individual performance outcomes, including:
- Regulatory and documentation compliance (timely assessments, care plans, SDRs, grievances, incident documentation).
- Performance management of all staff on key indicators benchmarks, as defined in performance addendum.
- Housing, safety, and SDOH outcomes including risk mitigation and reduction of unresolved high‑risk concerns.
- IDT integration and participant experience, reflected through positive feedback on collaboration and communication.
- Team performance, engagement, retention, and completion of performance reviews.
- The Supervisor’s own caseload documentation quality, timeliness, and modeling of trauma‑informed, person‑center practice.

Qualifications

REQUIRED
Education
- Master’s degree from a school of social work accredited by the Council on Social Work Education
Licensure, Certification, Registration or Designation
- Hold minimum state certification/registration (e.g., LSW) for master’s level social worker
- REQUIRED LICENSURE for NEW MEXICO: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) from The New Mexico Board of Social Work-LMSW License
- Meets all state requirements for practicing social work in healthcare settings (including supervision where applicable)
- Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
- CPR/BLS and First Aid as required by state
Work Experience and Qualifications
- Two (2) years’ supervising staff
- Three (3) years’ social work experience in medical/healthcare setting conducting assessments, care planning, and case management
- One (1) year experience with complex geriatric/adult populations with multimorbid medical and psychosocial conditions
- One (1) year experience identifying and responding to health and safety issues, housing instability, and social determinants of health
- Two (2) years’ experience conducting psychosocial assessments, risk assessment, and developing care plans
- Knowledge of state, county, and local governmental and community-based resources.
- Extensive knowledge of Medicare, Medicaid/Medi-Cal, and dually eligible benefits and services
- Familiarity with medical terminology, common conditions, common medications, experience with older adults with complex multimorbid medical issues
PREFERRED
- Five (5) years in Social Services
- Three (3) years’ experience with complex geriatric/adult populations with multimorbid medical and psychosocial conditions
- Three (3) years’ supervising staff
- Experience working in an IDT environment
- Bi-lingual
