Behavioral Health Care Manager

Manage ECM members with behavioral health needs, primarily Tier 1 and Tier 2 members Conduct comprehensive biopsychosocial assessments both telephonically and in person Develop therapeutic relationships that promote engagement, self-management, and improved health outcomes Utilize evidence-based interventions including Motivational Interviewing, Behavioral Activation, solution-focused techniques, and brief therapeutic interventions Provide crisis assessment and facilitate warm handoffs to crisis response resources when appropriate Collaborate with primary care providers, specialists, behavioral health providers, hospitals, health plans, and community organizations Participate in Interdisciplinary Care Team (ICT) meetings, Systematic Case Reviews (SCRs), and case conferences Requirements Master's degree in social work, counseling, psychology, or another related behavioral health field Current unrestricted CA LCSW, ACSW, LMFT, AMFT, LPCC, APCC, or equivalent licensure/registration as permitted by program requirements Minimum of 2+ years of behavioral health, care management, case management, or community-based experience Current and valid Driver's License with a clean driving record Reliable transportation and proof of auto insurance Secure home workspace with reliable internet connection.
