Social Worker

Medical Social Worker (LMSW/LCSW) We are seeking a compassionate and motivated Medical Social Worker to join a fast-paced hospital setting supporting patient care and discharge planning. This role is ideal for an LMSW or LCSW who is comfortable working directly on a hospital unit alongside a multidisciplinary team. New LMSWs are encouraged to apply if they are confident working in an acute care environment rather than a traditional office setting. Responsibilities Provide social work support to patients within a hospital/unit setting Manage discharge planning for approximately 25 patients per day Collaborate closely with Nurse Care Managers (NCMs), physicians, and interdisciplinary teams to coordinate safe transitions of care Assess patient psychosocial needs and identify appropriate community resources and support services Document interventions and maintain accurate patient records Participate in multidisciplinary treatment planning and care coordination Opportunity to work within pediatric populations if interested Requirements Active LMSW required (LCSW strongly preferred) Comfortable working in a hospital or inpatient setting Strong communication, organizational, and care coordination skills Ability to manage a high patient volume in a fast-paced environment Preferred Qualifications Minimum of 2 years of experience in medical social work, mental health, or human services as part of a multidisciplinary treatment team Prior hospital or inpatient experience strongly preferred Candidates with outpatient or nonprofit-only backgrounds may not be the best fit for this role Schedule Monday – Friday 8:30 AM – 4:30 PM or 9:00 AM – 5:00 PM This is an excellent opportunity for a social worker looking to grow within a collaborative hospital environment while making a direct impact on patient outcomes and continuity of care.
