Registered Nurse - Acute Care Case Management

Assignment Duration

This is a 13-week contract position.

Schedule

Day Shift: 5x8 Hour schedule (07:30 - 16:00), including alternating weekends.

Job Description

We are seeking an experienced Registered Nurse with acute care experience to join our dedicated team in case management. The ideal candidate will have a strong background in discharge planning and care coordination, working across multiple units within a Level II Trauma Center. The role requires collaboration with a multidisciplinary team to enhance patient care and facilitate smooth transitions.

Responsibilities

• Provide comprehensive case management focused on patient assessment, planning, and coordination of care.
• Collaborate with healthcare providers across various units, including Med Surg and Emergency Department.
• Conduct discharge planning to ensure timely and effective transitions of care for patients.
• Utilize AIDA EMR and Epic charting systems for accurate documentation and communication.
• Participate in multidisciplinary meetings to discuss patient care and improvement strategies.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care nursing experience required.
• Previous travel experience preferred.
• Current BLS certification required.
• Washington State RN license required; compact license must be in hand at submission.
• Experience with Epic and AIDA EMR preferred.
• Strong preference for candidates with diverse case management experience.
