Housing Specialist - Case Manager

A faith-based nonprofit organization in Louisville is seeking a compassionate Housing Specialist to provide individualized support to individuals experiencing homelessness. This vital role combines housing navigation, advocacy, care coordination, and faith-based service to help clients overcome barriers to housing and work toward long-term stability and independence. Key Responsibilities Conduct comprehensive assessments of client needs, including housing, health, income, and personal goals. Develop individualized housing and service plans in collaboration with clients. Help clients navigate housing applications, shelters, government programs, and community resources. Connect clients with healthcare, mental health, recovery, employment, and other supportive services. Advocate for clients with landlords, housing programs, government agencies, and service providers. Provide crisis intervention and de-escalation using trauma-informed and harm-reduction practices. Support clients in developing life skills, financial literacy, and self-advocacy skills. Monitor client progress and adjust service plans to promote long-term stability. Collaborate with healthcare providers, outreach workers, housing specialists, and community partners. Maintain accurate case notes, service plans, progress documentation, and program records. Help develop a Learning Center by identifying client needs and coordinating educational opportunities in employment readiness, household management, and life skills. Qualifications Bachelor’s degree in Social Work, Psychology, Human Services, or a related field preferred. 1–3 years of experience in housing services, case management, social services, homeless services, or behavioral health. Experience working with individuals impacted by homelessness, mental illness, substance use, and trauma. Knowledge of trauma-informed care, harm reduction, strengths-based approaches, housing systems, public benefits, and community resources. Strong communication, advocacy, organization, and crisis-management skills. Ability to work independently and collaboratively in a fast-paced environment.
