Responsibilities
- Administer medications and treatments per physician orders, and document outcomes
- Monitor patient status, perform routine assessments, and report changes to the supervising RN
- Provide ongoing nursing care, including vital signs, wound care, and symptom management
- Assist residents with activities of daily living (ADLs) and promote comfort and dignity
- Maintain accurate charting in the electronic medical record; follow infection control procedures
- Collaborate with the care team to ensure safety, quality of care, and compliance with regulations

Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Experience in long-term care or geriatric care preferred
- Ability to work all shifts and adapt to a travel assignment
- Strong communication, observation, and critical-thinking skills
- Ability to meet onboarding and assignment requirements
