Job Description

At Mary Free Bed at Home, we're looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health Clinical Resource Therapists (Physical Therapists) are responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, and provision of direct therapy care within the policies and standards of the Agency. In partnership with Mary Free Bed Rehabilitation Hospital, Mary Free Bed at Home is an accredited recognized leader in home health. We deliver industry-leading therapy and nursing expertise and services that exceed national benchmarks throughout the West Michigan service area.

This position supports patients in the southern Grand Rapids and surrounding areas, and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

We Are Looking For Compassionate Physical Therapists With

• Must possess at minimum a Master's degree from an accredited Physical Therapy school.
• A minimum of two (2) years of experience working as a Physical Therapist including experience in home care setting is preferred.
• Must have one year home care experience
• Licensed Physical Therapist issued by the state in which you will be working.
• Current driver's license and ability to driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
