Responsibilities:

• Provide high-quality patient care to a diverse population of residents, including those with complex medical conditions and rehabilitation needs
• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
• Monitor and report patient progress, and modify care plans as needed
• Collaborate with other healthcare professionals to ensure seamless transitions and comprehensive care
• Participate in quality improvement initiatives and contribute to ongoing professional development
• Maintain accurate and detailed records of patient care and treatment
