Our care model emphasizes longer visits, proactive outreach, and strong provider–patient relationships to improve outcomes and reduce unnecessary emergency room utilization. Clinics are located in accessible, retail‑style settings rather than traditional medical office complexes, creating a modern, patient‑friendly environment and a collaborative workspace for clinical teams. This is a newly created role designed to expand acute care access within a value‑based primary care model. Position Overview We are seeking an experienced Acute Care Nurse Practitioner (ACNP) to support acute and urgent patient needs while collaborating closely with Primary Care Providers to maintain continuity of care. This role blends acute care with outpatient primary care responsibilities and is ideal for a provider who enjoys variety, clinical autonomy, and working within a team‑based, patient‑centered model.
