Pleasant Hill Post Acute is seeking a compassionate dependable and clinically skilled Registered Nurse (RN) to join our team on a Part-Time / On-Call basis .

The ideal candidate is committed to providing high-quality resident-centered care and working collaboratively with residents families physicians and the interdisciplinary healthcare team.

Responsibilities

Provide skilled nursing care to residents in accordance with individualized care plans.
Administer medications and treatments as ordered.
Monitor residents conditions and promptly report changes in health status.
Perform nursing assessments and assist with care planning.
Maintain accurate and timely clinical documentation.
Communicate effectively with residents families physicians and healthcare staff.
Follow infection control safety and facility policies and procedures.
Promote resident dignity comfort safety and overall well-being.
Respond appropriately to changes in residents conditions and emergency situations.

Qualifications

Current and valid California Registered Nurse (RN) license .
Previous skilled nursing or post-acute experience preferred.
Strong clinical communication and organizational skills.
Reliable compassionate and team-oriented.
Ability to work independently and effectively in a fast-paced healthcare environment.
