Pleasant Hill Post Acute is seeking a compassionate dependable and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a Part-Time / On-Call basis .

The ideal candidate is committed to providing safe high-quality resident-centered care while working collaboratively with residents families physicians and the interdisciplinary healthcare team.

Responsibilities

Provide nursing care and services to residents in accordance with established care plans.
Administer medications and treatments as ordered.
Monitor residents conditions and promptly report changes to the appropriate healthcare professionals.
Complete accurate and timely nursing documentation.
Assist with assessments and contribute to individualized care planning.
Communicate effectively with residents families and members of the healthcare team.
Follow infection control safety and facility policies and procedures.
Promote residents comfort dignity and overall well-being.

Qualifications

Current and valid California LVN license .
Previous skilled nursing or post-acute experience preferred.
Strong communication and organizational skills.
Dependable compassionate and team-oriented.
Ability to work effectively in a fast-paced healthcare environment.
