Licensed Practical Nurse (LPN)

Seeking a skilled and compassionate LPN to contribute to the exceptional care provided in a reputable skilled nursing facility with a focus on both short-term rehabilitation and long-term care. In this contract position, you will support a diverse patient population recovering from illness, injury, or surgical procedures, working alongside a multi-disciplinary team dedicated to enhancing quality of life.

Schedule Details:

• 8-hour night shifts, 40 hours per week
• Shift time: 11:00 PM 7:00 AM, Monday through Friday
• Every other weekend rotation
• No on-call or call-back required
• Holiday coverage: 75% of holidays required
• Floating required within assigned region; time is compensated

Preferred Qualifications:

• Active LPN license and current BLS certification
• Minimum 18 months' experience in a skilled nursing facility or long-term care rehab setting
• Proficiency with trach management (refresher provided), foley care, wound care, dressing changes, IV starts and management, tube feedings
• Experience with Epic EMR, Alaris IV pumps, and Omnicell medication dispensing is helpful
• Candidates must have a permanent address more than 50 miles from the primary work location
• Ability to float to regional facilities as needed

Key Responsibilities:

• Deliver high-quality nursing care to adults with diverse medical needs, including post-stroke, amputation, neurological recovery, and orthopedic rehabilitation
• Support admissions processes (approx. 12 daily) in coordination with administrative nursing staff
• Collaborate across disciplinestherapy, case management, and leadershipto ensure continuity in patient care
• Maintain complete and timely documentation in the electronic medical record system
• Adapt to patient ratios of approximately 1:26 during night shifts
