Travel Physical Therapist - Home Health

American Traveler is seeking a Physical Therapist with Home Health experience for a 13-week travel contract serving a geriatric patient population across a defined territory in the Far East Bay Area of California.

Job Details

• Home Health setting serving a geriatric patient population,
• Territory covers the Far East region, spanning Discovery Bay to Pittsburg,
• Average patient load of 5-7 patients per day,
• Day shift schedule, Monday-Friday, 8:00 AM-5:00 PM (5x8-hour shifts),
• No on-call or weekend requirements,
• EMR: HCHB (experience required); OASIS experience preferred,
• iPad provided for documentation; mileage reimbursed at the IRS standard rate,

Job Requirements

• Minimum 1 year of recent Home Health PT experience required,
• Current BLS certification required,
• Active PT license required if applicable,
• HCHB EMR experience required,
• Must be willing to travel within a territory of up to 60 miles from the hub/office,
• Must be able to meet productivity standards,

Additional Information

• Travelers are expected to be flexible with territory assignments,
• OASIS experience is preferred but not required,
• First-time travelers are welcome to apply,
• Local candidates are accepted and eligible for the travel rate,
• Scrubs required in any color; jeans are not permitted,
• 2 professional supervisor references from within the past 2 years required for consideration,
• Full 7-year work history with all gaps explained required for consideration,
