Hospice Registered Nurse After Hours On Call / RN Position Type: Full-Time Coverage Area: Madison Heights / Detroit / Rochester Hills / Fort Gratiot / surrounding areas Pay: $80,000 - $86,000 annually, based on experience Schedule: After Hours On Call RN — 5 to 16 hour shifts per pay period (4:30 pm–8:30 am) with every other weekend rotation. Qualifications Current licensure as a Registered Nurse in the applicable state; associate degree preferred. CPR certification is recommended (required for direct patient care in Georgia). Prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Willingness to participate in an on‑call rotation and maintain reliable transportation, valid driver's license, and state-required liability auto insurance. Strong communication and organizational skills; proficiency in Microsoft Office Suite and EMR systems. Ability to collaborate effectively within an interdisciplinary team. Physical ability to lift patients over 200 lbs with assistance, transport supplies and equipment up to 20 lbs over distances of 500 feet, and perform frequent standing, walking, sitting, stooping, and reaching. Responsibilities Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintain timely and accurate documentation, ensure care plans reflect current patient status, and communicate effectively with the care team to coordinate optimal use of resources, especially in complex or high‑risk situations. Provide education and emotional support to patients and caregivers based on individual needs; actively participate in on‑call rotations, admissions, and care transitions as directed. When serving as a Care Manager, coordinate all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPNs, LVNs, and Hospice Aides; participate in IDG meetings and patient/family conferences. When serving in an On‑Call capacity, respond to triage calls, perform visits and assessments, coordinate care with the Administrator On Call, and may attend IDG meetings and family conferences; also participate in training, committees, and other duties as assigned within licensure scope.
