We are seeking a dedicated Registered Nurse (RN) with strong home health and OASIS experience to provide compassionate, skilled care in a dynamic, rewarding environment. This is a 13-week full-time contract, ideal for professionals confident in clinical assessment, timely documentation, and efficient time management.

Schedule & Assignment Details:

• Full-time: Monday through Friday
• On-call coverage may be required
• 13-week assignment with potential for extension
• Structured daily visit expectations

Key Responsibilities:

• Deliver high-quality, in-home nursing care and skilled interventions
• Independently conduct thorough patient assessments and accurately complete OASIS documentation within required timelines
• Collaborate closely with physicians and interdisciplinary care teams to formulate and implement individualized patient care plans
• Perform wound care, manage medications, provide patient/family education, and monitor for health status changes
• Communicate promptly with the clinical team regarding patient updates, changes in condition, and care needs

Qualifications:

• Current, active RN license
• At least 1 year of recent home health RN experience
• Proficiency with OASIS documentation is required
• HomeCare HomeBase experience strongly preferred
• Valid drivers license and reliable transportation for community-based visits
• Ability to balance a structured schedule with multiple patient visits per day
