Job Description Job Description Position Title: Licensed Practical Nurse (LPN) – Long Term Care (Days) Location: Meeker, Colorado Job Type: Contract Schedule 3 x 12-hour shifts per week Day Shift 36 guaranteed hours per week Holiday coverage required Position Overview "Navitas Healthcare, LLC" is an experienced Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care setting. The role requires strong clinical judgment, independence, and prior experience in skilled nursing or LTC environments. Key Responsibilities Provide direct patient care including bathing, feeding, and toileting Administer medications and treatments as prescribed Monitor patient conditions and report changes promptly Maintain accurate and timely documentation Assist with care planning and MDS documentation (preferred) Ensure compliance with safety and infection control standards Collaborate with interdisciplinary healthcare teams Required Qualifications Active, unrestricted Colorado LPN License Minimum 3 years of Long Term Care (LTC) experience Current BLS/BCLS certification Preferred Qualifications Experience with MDS documentation Experience using PointClickCare EMR Skills & Competencies Strong clinical judgment and patient care skills Ability to work independently in a long-term care environment Strong documentation and communication skills Team collaboration and resident-focused care approach For more details contact at View email address on click.appcast.io or Call / Text at View phone number on click.appcast.io.
