Physical Therapist – Outpatient

Job Description:

Seeking a dedicated Physical Therapist (PT) to evaluate patients, develop individualized treatment plans, and provide evidence-based outpatient physical therapy services. The therapist will focus on improving functional mobility, achieving patient-specific goals, and helping patients safely return to daily and community activities.

Responsibilities:

• Perform comprehensive patient evaluations and develop individualized treatment plans based on patient needs, goals, and physician orders.
• Establish measurable treatment goals and modify plans according to patient progress.
• Provide therapeutic interventions to improve mobility, strength, function, and independence.
• Deliver 60-minute evaluation and treatment sessions .
• Document evaluations, treatment plans, progress notes, functional outcomes, medical necessity, and charges accurately and promptly.
• Ensure treatment is provided under valid physician orders and within applicable insurance authorizations.
• Educate patients and families regarding treatment plans, exercises, and strategies for continued progress.
• Coordinate care with physicians and other healthcare professionals.
• Supervise and support students when assigned.
• Maintain professional licensure and comply with state practice acts, ethical standards, and regulatory requirements.
• Follow patient safety, infection prevention, safe patient handling, and workplace safety protocols.
• Participate in continuing education and quality improvement initiatives.

Required Qualifications:

• Degree in Physical Therapy from an accredited school
• Minimum 1 year of outpatient clinical experience
• Active, applicable state PT license
• CPR certification
