In this role, you will provide high-quality, resident-centered care while collaborating with physicians, nursing staff, and interdisciplinary team members to ensure the best possible outcomes for our residents.Key ResponsibilitiesProvide and oversee direct nursing care in accordance with physician orders and individualized care plansAdminister medications, treatments, and specialized nursing procedures safely and accuratelyAssess residents and monitor for changes in conditionCommunicate resident status updates to physicians, families, and appropriate team membersMaintain accurate and timely clinical documentationSupervise and support LPNs and CNAsRespond appropriately to resident needs, concerns, and emergenciesParticipate in care planning meetings and interdisciplinary team discussionsPromote resident dignity, comfort, safety, and quality of lifeEnsure compliance with all federal, state, and facility regulationsUtilize PointClickCare (PCC) and other clinical systems as neededQualificationsCurrent, unrestricted Registered Nurse (RN) license in South CarolinaCurrent CPR certificationMust maintain all required continuing education and licensure requirementsMust remain in good standing with the South Carolina Board of NursingExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) preferredStrong clinical assessment, leadership, communication, and organizational skillsWhat Makes You a Great FitCompassionate and resident-focusedStrong clinical judgment and critical thinking skillsDependable and professionalExcellent communication and teamwork abilitiesPassionate about delivering exceptional care to an underserved populationApply TodayJoin West Village Post Acute and become part of a team that's redefining post-acute care.
