Director of Social Work - LTC located in Suffolk County, NY
Salary: Negotiable Based On Experience

Qualifications:
Must have LMSW or LCSW
Must have 3+ years of experience as a Director of Social Services in long-term care
Must have experience in long-term care
Must have strong organizational and case management abilities
Strong leadership skills
Excellent communication and interpersonal skills
Responsibilities of the Director of Social Work-Nursing Home:
Plans and directs a comprehensive social services program which provides support services for residents and families.
Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
Participates in an interdisciplinary team approach to better serve the needs of the facility population.
