Position Overview


CVH Care Home Health is seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to provide high-quality care in a home health setting. This role involves working directly with patients across designated territories, supporting their rehabilitation and improving mobility and overall function.


The ideal candidate is adaptable, patient-focused, and comfortable working independently in a field-based environment.

Service Areas


Assigned territories include, but are not limited to:

• Area 2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
• Area 1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City

Travel between patient locations is required.

Key Responsibilities

• Provide physical therapy treatments in accordance with established care plans
• Assist in improving patient mobility, strength, and functional independence
• Accurately document patient visits, progress, and outcomes
• Communicate effectively with supervising Physical Therapists and interdisciplinary teams
• Educate patients and caregivers on therapy techniques and safety practices
• Maintain compliance with home health standards and regulatory requirements

Schedule & Work Structure

• Day shifts
• 13-week contract assignment
• Weekend coverage may be required
• Holiday coverage required

Two scheduling structures are available:


Guaranteed Hours Option:

• Weekly productivity expectations must be met
• Flexibility to travel outside assigned areas or work weekends if needed
• Guarantee conditions apply based on assignment guidelines

Non-Guaranteed Hours Option:

• Flexible scheduling based on patient volume
• Ability to accept or decline patient assignments
• Opportunity to expand caseload across service areas

Qualifications

• Minimum 1 year of professional experience as a Physical Therapy Assistant
• Home health experience preferred
• Active California PTA License
• Valid Driver's License and reliable transportation
• Current auto insurance and registration

Required Certifications & Compliance

• BLS (Basic Life Support)
• TB test (annual)
• Immunizations: MMR, Varicella, Hepatitis B, Tdap (within 10 years), Influenza (annual)
• Drug screening and physical exam clearance

Additional Information

• Mileage reimbursement provided between patient visits (excluding commute)
• Candidates must be willing to treat a variety of patients, including those with infectious conditions as required
