Work Environment

This role requires a physical presence in patients' homes, allowing you to assess and treat them in a comfortable, familiar environment. Your work schedule will be Monday to Friday, from 2 PM to 8 PM, enabling you to provide care during times that best suit your patients' routines. You will engage with a diverse patient population, which will enhance your professional experience and skillset.

Key Responsibilities

• Deliver comprehensive assessment and treatment plans through a thorough evaluation of patients' physical and functional capabilities.
• Collaborate with physicians and the patient care team to develop individualized care strategies aimed at achieving measurable health goals.
• Implement therapeutic interventions, including but not limited to exercise prescription, manual therapy, and modality application to facilitate recovery.
• Instruct patients and their caregivers on therapeutic exercises, the use of assistive devices, and rehabilitation techniques to ensure continuity of care outside of therapy sessions.
• Monitor patient progress and adjust treatment plans accordingly, ensuring alignment with medical directives and ethical standards.
• Organize and prioritize caseload effectively, coordinating with team members to ensure optimal patient care and timely interventions.
• Maintain meticulous documentation of patient interactions and treatment outcomes, adhering to all regulatory requirements.

Patient Population / Client Focus

Your primary focus will be on adult and pediatric patients who require physical rehabilitation services. You will assist individuals recovering from surgery, managing chronic conditions, or adjusting to changes in mobility and function. Each patient presents a unique set of challenges, making your adaptability and expertise crucial for their recovery.

Qualifications

• A valid and current license to practice as a Physical Therapist in Florida.
• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Cardiac Life Support (BCLS) certification, with a requirement to obtain within 30 days of your employment start date.
• While no prior experience is required, a willingness to learn and a passion for patient-centered care is essential.

Preferred Experience

• Experience in home health care or outpatient therapy settings is advantageous but not mandatory.
• Familiarity with electronic medical records (EMR) and patient management software will be beneficial to streamline documentation and reporting.

Compensation and Benefits
