Compensation: $75,000 - $110,000 annualized (per-visit model based on volume)

We operate alongside Interim Healthcare, Lifespark, Adara, Aveanna, and Gentiva , but our focus is simple.

Support the nurse so the nurse can focus on the patient.

What You'll Do
•Provide skilled nursing visits in homes and community settings.
•Perform assessments, wound care, and medication management .
•Educate patients and families on care plans and recovery.
•Collaborate with RN Case Managers and interdisciplinary teams.
•Float to nearby territories as needed to support census and coverage.
•Maintain accurate, timely documentation .

What We're Looking For
•Active Minnesota Registered Nurse (RN) license.
•Strong clinical judgment and ability to work independently.
•Experience in home health preferred, but not required.
•Comfortable in a field-based, travel-focused role .
•Reliable transportation.
