What You'll Do
•Provide skilled nursing visits in homes and community settings.
•Perform assessments, wound care, and medication management .
•Educate patients and families on care plans and recovery.
•Adapt quickly to new territories and patient needs.
•Collaborate with RN Case Managers and interdisciplinary teams.
•Maintain accurate, timely documentation .

What We're Looking For
•Active Minnesota Registered Nurse (RN) license.
•Strong clinical judgment and willingness to learn.
•Comfortable working independently in a field-based role .
•Adaptable and confident in new environments.
•Reliable transportation.
