Social Worker Albany, OR Specialty: Social Work Position Type: Travel Contract Length: 13 weeks Pay: $1711 - $2062 | Shift: 5x8 Days Are you a compassionate Social Worker looking to make a meaningful impact in hospice care? Do you thrive in providing support to patients and their families during critical times? Our healthcare partner is seeking dedicated professionals like you to join their team in providing exceptional care in the home setting. As a Social Worker in this role, you will be an integral part of a multidisciplinary team, offering essential services to patients in various home settings. Your expertise will guide patients and families through the final stages of illness, ensuring they receive the support and care they need.
- Provide intermittent skilled social work visits and support
- Collaborate with nursing staff for pain and symptom management
- Offer counseling, spiritual support, and bereavement services
- Utilize personal automobile for travel to patient locations
- Maintain compliance with licensure and insurance requirements
- Oregon LMSW or CSWA licensure required
- Healthcare Provider Level BLS required
- Oregon LCSW preferred
- Experience as a social worker in a hospital or medical setting preferred
- Need 1 year experience in setting
