Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician’s orders for each patient while adhering to confidentiality standards and professional boundaries. Essential Job Functions Deliver and coordinate quality patient care in compliance with physician orders. Continuously observe and assess patient condition and care needs, reporting changes to the supervisor or physician. Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner. Participate in, implement, and update the nursing care plan. Take appropriate nursing action based on assessment to achieve expected outcomes. Recognize changes in patient needs and respond with interventions to prevent or reduce risk. Maintain compliance with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Use infection control measures that protect staff and patients according to OSHA standards. Educate patients and families about disease processes, self‑care techniques, and prevention strategies. Participate in educational programs, continuing education units, internal learning management, and skill evaluations. Qualifications Active, unencumbered LPN/LVN license in the state of practice. Current CPR certification (hands‑on component). Aveanna can assist with obtaining certification after hire if necessary. TB skin test current within the last 12 months. Reliable transportation. Six months prior hands‑on nursing experience preferred but not required. Additional State‑Specific Requirements South Carolina – One year of pediatrics experience. California – One year of experience working under a current nursing license. Louisiana – One year of experience working as a licensed nurse. Continuing education as required by state. Preferences Six months of recent experience as a Licensed Nurse in a clinical care setting. Home‑health experience. Other Skills/Abilities Attention to detail. Time management. Effective problem‑solving and conflict resolution. Good organization and communication skills. Physical Requirements Speak, read, write, and understand English. Travel as needed. Lift up to 50 pounds. Reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Sit and climb stairs. Vision and hearing acuity. Sensible smell and touch. Respond physically and mentally to emergency situations in the home or during transport. Environment Work in a variety of environments that may involve exposure to allergens, blood, bodily fluids, and infectious diseases.
