Physical TherapistWe are seeking a licensed Physical Therapist to provide high-quality rehabilitation services to patients throughout the Morris County region. The ideal candidate will be experienced in patient evaluations, treatment planning, therapeutic interventions, patient education, and clinical documentation while working independently in a home health environment.Key ResponsibilitiesPerform comprehensive patient evaluations and functional assessmentsDevelop individualized treatment plans and rehabilitation programsImplement therapeutic interventions to improve mobility, strength, and functional independenceEducate patients and caregivers regarding treatment goals, home exercise programs, and safety measuresDocument evaluations, treatments, progress notes, and discharge summaries accurately and timelyCollaborate with physicians, nurses, case managers, and interdisciplinary healthcare teamsMaintain compliance with clinical, regulatory, and documentation standardsSupport quality improvement and patient outcome initiativesRequired QualificationsActive New Jersey Physical Therapist LicenseGraduate of an Accredited Physical Therapy ProgramAbility to work independently with strong clinical judgmentExcellent communication and patient care skillsDiverse Lynx LLC is an Equal Employment Opportunity employer.
