We look forward to you joining our growing family! This position is contingent and paid on a per visit basis; orientation and additional training will be paid at a separate hourly rate. We offer competitive wages, bi-weekly pay, and direct deposit. Responsibilities: Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments Perform assessments within 24-48 hours of receiving the initial referral Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary Educate patients and caregivers in a way they will understand Record on a timely basis all evaluation data, treatments, and patient#39;s response to therapy intervention Supervise Physical Therapy Assistants as per agency policy Participate with staff, patients, and physicians in discharge planning activities Adhere to Federal, State, and Accreditation requirements including Medicare and Medicaid regulations Minimum Qualifications: Physical Therapist License in good standing within the State of Michigan Preferred one (1) or more years of PT experience, specifically in a community/home health care setting Must have knowledge of Medicare/CMS guidelines Must have experience with Outcome and Assessment Information Set (OASIS) assessments Experience using an EMR (Kinnser preferred) Must have your own reliable transportation, valid MI driver’s license, with proof of auto insurance BLS through AHA or Red Cross #J-18808-Ljbffr No Place Like Home Health LLC
