LPN – Home Health

Schedule: 8hrs

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 12

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Ability to use electronic medical charting and time-keeping software
• Proof of current Hepatitis consent/declination and TB vaccination record
• Reliable transportation and current auto liability insurance coverage
• Must read, write, and speak fluent English
• Good and regular attendance

Responsibilities:

• Administer skilled nursing care to patients in their place of residence.
• Assist in identifying the patient's physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing care with an interdisciplinary team.
• Provide effective communication to patient/family and colleagues.
• Assist the patient/family in learning appropriate self-care techniques.
• Utilize infection control measures in providing care.
• Assist with end-of-life care for hospice patients.
• Support the admitting, discharging, and transferring of patients.
• Participate in annual in-service and performance improvement programs.
