Care Management Coordinator – Case Management at Covenant Medical Center (Lubbock, TX) The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health‑care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care. This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously. Shift Per diem / On‑call, Variable shift Please note that this position is onsite and therefore requires the ability to commute to Covenant Medical Center in Lubbock, TX. Pay Range: $17.12 - $26.18 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
