Job Description This posting is for multiple openings of Care Management Coordinator - Case Management at Covenant Medical Center in Lubbock, TX. We are hiring for the following opportunity: Full Time, Day Shift, 40 hours a week. This position is onsite and requires the ability to commute to Covenant Medical Center in Lubbock, TX. The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies, and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided, and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation, and support the delivery of quality patient care. This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously. Responsibilities Coordinate communication among patients, health care providers, insurance companies, and other stakeholders. Ensure safe and efficient transitions of care and regulatory compliance. Maintain accurate records and process documentation. Support the delivery of quality patient care. Required Qualifications 2 years of administrative experience, preferably in a healthcare setting or demonstrated knowledge of healthcare terminology. Preferred Qualifications Bachelor's Degree or higher in Business, Healthcare, or related field.
