Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Unity Health and Rehabilitation Center. As a Full-Time Registered Nurse, you will play a vital role in providing exceptional patient care to our residents. In this position, you will work closely with our interdisciplinary team to deliver top-notch medical care, empowering our patients to achieve their rehabilitation goals.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing wound care, and monitoring vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct ongoing assessments to identify patient needs and make necessary referrals
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with therapy teams to develop and implement comprehensive rehabilitation plans
