Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Ohio Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Home Health experience is required.
Experience with HCHB (Homecare HomeBase) electronic documentation is required.
Ability to manage a caseload consistent with weekly productivity expectations (points-based productivity; client indicates 32 points weekly).
Valid driver’s license and reliable transportation for travel within assigned territory (Guernsey County and eastern Muskingum County).
Preferred: prior territory-based home health experience and comfort with community-based care coordination.

Shift: Days.
Shift Details: Monday–Friday, 8:00 AM–5:00 PM.
Position Type: Full-time, 40 hours per week.
Territory: Guernsey County and east Muskingum County. Must be able to travel within this territory.
