Physical Therapist - Travel Contract

Minimum Qualifications
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Ohio Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Home Health experience required.
Experience with HCHB (Homecare Homebase) required.
Availability to work within the assigned territory covering Perry and Morgan counties.

Preferred Qualifications
Previous travel or per-diem home health experience preferred.
Experience managing a caseload of point-based visits preferred.

Assignment Details
Shift pattern: Monday through Friday, 40 hours per week.
Shift length: 8 hours per day.
Territory includes travel within Perry and Morgan counties.
Points-based weekly caseload: 32 points per week.
