Physical Therapist (PT) - Take Your Therapist Home Program

Join us at Rivetus Rehab! Take Your Therapist Home As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program! This role is focused on Home Health Care in the Ann Arbor and surrounding areas with a steady referral source from multiple skilled nursing facilities. This role includes PTA supervision and provides the opportunity to pick up additional time at nearby SNF.

In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is an unique program where clinicians work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes! Rivetus has been serving the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location: Ann Arbor 4701 E. Huron River Drive Ann Arbor, MI 48105. This is a home care role providing Home Health therapy services with the option to have additional time in the SNF. Home Health geographic area is the vicinity around Ann Arbor.

Schedule: Full Time, Monday-Friday or Tuesday-Saturday (Tues-Sat schedule will receive premium shift pay).

Required Qualifications:

• Successful completion of an accredited Physical Therapist program.
• Licensure as required by the State of Michigan.

Skills and Abilities:

• Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
• Initiative and flexibility in the work environment.
• Ability to react effectively to the needs of patients and their families.
• Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
