Case Manager Lead

Job Summary The Lead Case Manager will coordinate, monitor, and mentor the case management team to ensure all services meet client needs. Responsibilities include assessing and re‑assessing household needs, educating families on community resources, developing housing stability plans, scheduling appointments, and providing follow‑up to maintain progress. The role balances administrative duties with field outreach. Responsibilities Assign and monitor team caseload, reviewing daily referrals and screening potential clients. Designate clients to case managers for intake and verify supporting documents. Provide information and referrals to callers and walk‑ins. Conduct comprehensive screenings and assessments—collecting functional, environmental, financial, employment, housing, educational, and health information—to develop case plans. Develop support systems, coordinate and facilitate move‑in activities, negotiate rent, and arrange service referrals as needed. Conduct home and field visits; perform crisis intervention when necessary. Monitor case managers’ quality and effectiveness of services; review cases with the Program Coordinator and team. Audit case files regularly for completeness and accuracy. Assist in coordinating services for veterans and families to promote self‑sufficiency and family stabilization. Make referrals to VA/community providers for therapeutic, social, educational, and medical resources. Support case managers, clients, and families through ongoing conferencing, counseling, and advocacy. Assure all program assets and documents are maintained and secure. Develop and conduct workshops related to program services and veterans’ needs. Participate in outreach events. Perform other duties as assigned. Lead the team through detailed action plans to meet client goals and find necessary resources. Ensure intake, assessments, service planning, and delivery meet all documentation requirements for provided services. Collaborate with other service providers to link participants to community resources and supportive services. Conduct weekly case conferencing and monthly file audits to assure participant records are complete and accurate. Mandated Reporter All employees are considered mandated reporters and must report any instance of physical or sexual abuse to the proper authorities. Requirements Pass a fingerprint clearance, background check, criminal history, and employment and education verifications. Ability to perform under emotional stress and conflict with difficult, uncooperative, or potentially aggressive clients. Associate’s Degree in Social Work or related field, or a combination of education and two years of relevant experience. Complete VOALA Case Management training within six months of hire. Minimum of two years of experience as a case manager. Proven leadership ability. Preferred Qualifications Bachelor’s Degree in Social Work or related field. Three years of experience working with the target population. Experience in a supervisory or lead role. Proficiency with HMIS navigation. Equal Opportunity Employer.
