Job Description The Good Seed is looking for a Lead Case Manager to join our dynamic team! The primary function of this position is client‑centered case management, support, engagement, and document facilitation. The ideal candidate will be a passionate quick‑learner, who thrives in fast‑paced work environments and possesses the ability to maneuver through challenges with a creative spirit. The Lead Case Manager will have general oversight and coordination of other case management. The Good Seed is a growing non‑profit organization where there is growth potential. Oversee all services and operational activities; Provide comprehensive case management that includes, but is not limited to: screening and assessing clients; creating, implementing and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing and other community resources; Foster a professional and welcoming environment in all interactions with clients, guests, vendors and colleagues, regardless of differing identities, backgrounds, beliefs and choices; Manage and evaluate the performance of assigned staff; Develop and maintain professional and supportive relationships with staff and subordinates; Provide information regarding services and transportation as needed to people seeking services; Work in tandem with case managers to assist clients in reaching their case plan goals; Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed; Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with established guidelines; Actively participate in client‑ and program‑related meetings and trainings; Adhere to strict boundaries and professional ethics in the care of others; Ensure that the facility is clean, free of potential hazards and safe at all times; Enforce house rules and policies; Qualifications 4‑year degree in human services, or comparable combination of education/work related experience in a social service setting; working knowledge of case management and planning techniques; Experience working with homeless individuals, youth (age 16‑25), and the continuum of care in a similar environment with the population of clients we serve (Preferred); Detail‑oriented and highly organized; Provide own transportation for work‑related travel as necessary; Maintain a clean driving record and valid driver’s license; CPR/First Aid Certified; Available to work M‑F, 8:00 a.m. to 4:00 p.m.; Skills/ability Leadership and management; Make sound decisions and has experience handling highly sensitive and confidential information; Multi‑task in a fast‑paced work environment; Quick to respond and work effectively in crisis situations, including the ability to de‑escalate volatile situations and apply effective conflict resolution techniques; Proficient with Microsoft Word and Excel; Excellent written and verbal communication skills; Attributes Patient, inquisitive, resourceful and flexible; Physical Ability Must be able to reach, bend, walk, stand and sit for long periods of time and climb stairs; Must be able to lift a minimum of 30 lbs.; Full‑time Additional Information All your information will be kept confidential according to EEO guidelines. #J-18808-Ljbffr
