Assess patient conditions, monitor vital signs, administer prescribed medications, manage wound care, and implement care plans in collaboration with the interprofessional team. Document assessments, interventions, and outcomes accurately in the medical record; ensure compliance with regulatory and payer requirements. Educate patients and families on disease management, medications, equipment use, and safety to promote optimal functional outcomes and self-care. Coordinate with supervisors, physicians, and case managers to adjust plans of care as patient needs evolve; facilitate transitions across care settings when appropriate. Maintain a safe and therapeutic environment, adhere to professional standards and agency policies, and participate in performance improvement activities.
