About the Facility

A small 20-bed skilled nursing facility in southern Middle Tennessee is seeking an experienced Interim Nursing Home Administrator to provide leadership and operational oversight.

The facility maintains an average daily census of approximately 15 residents , creating a smaller, relationship-driven environment where the Administrator can remain highly visible and engaged with residents and staff.

The community has several important strengths, including:

• 5-star CMS Quality Measures rating
• 3-star Staffing rating
• No current reliance on agency staffing
• A manageable 20-bed environment
• PointClickCare (PCC) electronic medical records

The facility's most recent survey occurred in September 2025 , resulting in six tags compared with a state average of 5.3, with no high-level deficiencies .

About the Interim Role

We're looking for more than someone who can simply keep the Administrator's seat occupied.

The ideal Interim NHA will bring strong operational leadership, corporate healthcare experience, and a collaborative approach to the facility.

You'll work closely with senior operational leadership and other organizational partners rather than operating independently or in a silo. Success in this position requires someone who is comfortable communicating across departments, following organizational processes, and making decisions as part of a broader leadership team.

For an Administrator accustomed to a more structured corporate environment, this position offers the opportunity to make an immediate impact in a smaller rural healthcare setting.

What You'll Do

• Provide day-to-day leadership and operational oversight for the skilled nursing facility.
• Maintain compliance with applicable state and federal regulations.
• Ensure the facility remains prepared for regulatory surveys and inspections.
• Work closely with nursing leadership and department heads to maintain quality resident care.
• Monitor operational, clinical, staffing, and financial performance.
• Maintain strong communication with corporate and executive leadership.
• Collaborate with organizational partners rather than operating departments independently.
• Support department leaders and establish clear expectations for performance.
• Maintain a visible leadership presence throughout the facility.
• Monitor census and support appropriate admission and occupancy strategies.
• Ensure timely follow-up on operational and regulatory concerns.
• Review quality and performance data and identify opportunities for improvement.
• Maintain and build upon the facility's strong quality-measure performance.
• Support staff retention and continue the facility's ability to operate without agency staffing.
• Promote a culture of accountability, teamwork, communication, and resident-centered care.
• Provide stability and continuity throughout the interim leadership transition.

You'll Be a Great Fit If You:

• Hold an active Nursing Home Administrator license applicable to Tennessee .
• Have previous experience successfully operating a skilled nursing or long-term care facility.
• Have worked within a corporate, regional, or multi-facility healthcare organization .
• Understand the importance of collaboration with corporate and executive leadership.
• Are comfortable working within established organizational structures, policies, and processes.
• Have experience with survey management and regulatory compliance.
• Can quickly assess facility operations and identify priorities.
• Communicate effectively with nursing leadership, department heads, residents, families, and executive leadership.
• Lead through influence, collaboration, and accountability rather than operating in a silo.
• Are comfortable working in a small, rural healthcare environment .
• Are interested in and comfortable with the lifestyle associated with a long-term rural community.

Particularly Valuable Experience

Candidates with experience working in a rural acute-care or healthcare environment will be especially attractive for this opportunity.

We're looking for an Administrator who understands that operating successfully in a rural setting can require a different leadership approach. Relationships matter, resources may be different from those available in a large metropolitan market, and leaders often need to work closely across departments to solve problems.

Experience with PointClickCare (PCC) is also preferred.
