Job Type Full-time Description This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as “medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs” (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, HCBS settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities. HCBS Coordinators provide leadership and support to a team that is laser-focused on delivering the best care possible. As a core team member, you’ll be responsible for monitoring and coordinating services, developing strategies to help individuals meet and exceed their goals, maintaining accurate and up-to-date records, and ensuring decisions and services are implemented in compliance with rules and regulations from governing bodies. You’ll emphasize individual choice and decision-making while collaborating with team members, individuals served, and support systems to create meaningful outcomes. What Winning Looks Like Staffing & Supervision: Hire, schedule, and supervise Direct Support staff; manage training, evaluations, meetings, documentation, and ensure adequate coverage including on-call and filling in as needed. Quality & Compliance Oversight: Ensure all services follow HCBS philosophy, agency policies, DHS regulations, and compliance standards; conduct site visits and maintain quality assurance across programs. Individual Service Coordination: Develop, implement, and monitor Individual Service Plans with case managers and IDT; ensure assessments, goals, authorizations, and required documentation are current and accurate. Medication, Finances & Daily Operations: Oversee medication administration processes, appointment follow-ups, and individual financial management including budgets, ledgers, and payee responsibilities. Communication & Administrative Leadership: Serve as primary liaison with case managers, families, and stakeholders; manage incident reports, service documentation, transportation coordination, and general operational duties including reports, committees, and agency support tasks. Age Requirement: This position requires you to be at least 18 years of age. Driver’s License: You’ll be responsible for transportation at times, so we need to trust your ability to be safe behind the wheel. Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this. Education Requirement: Minimum Education Requirement of High School Diploma/GED equivalent. Physical Demands May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment. May require prolonged sitting and working at a computer for up to eight hours. May require frequent bending, stooping, and stretching. May require prolonged standing for up to eight hours. Must wear hearing and eye protection in required areas. Must have ability to see objects/persons at a distance. Must have ability to see close work such as typed or handwritten material. Must have ability to hear conversations in a quiet and a noisy environment. Must have ability to determine where a sound is coming from. Must have ability to hear differences among bells, buzzers, beeps, horns, etc. Must have ability to communicate through speech. Must remain awake during working hours, if applicable.
