Job Description:

The RN Case Manager is responsible for case management and care coordination, working with a multidisciplinary team to ensure appropriate patient care. This role involves completing initial assessments, discharge planning, and utilization review, utilizing Interqual and Cerner systems in an acute care setting.

Responsibilities:

• Manage and coordinate patient care across all areas of the hospital
• Perform initial assessments and discharge planning
• Utilize Interqual criteria for patient evaluations
• Collaborate with healthcare team to ensure effective communication and patient care
• Adhere to 8-hour work shifts, including every other weekend as required

Requirements:

• BSN required
• Valid nursing license
• Prior recent experience with California Dignity facilities is mandatory
• BLS Certification
• Experience with all payers, uninsured, and homeless populations
• Proficiency with Cerner and Interqual systems
