Care Transition Specialist The Department of Public Health and Human Services (DPHHS) Behavioral Health and Developmental Disabilities Division seeks a Care Transition Specialist to provide Critical Time Intervention (CTI) services for low‑income Montanans transitioning from Montana State Hospital to the community. The role is client‑facing, takes place in the community, and requires frequent in‑state travel and flexible scheduling. Responsibilities Serve as a client‑facing case manager for individuals in the CTI program. Collaborate with community providers, family members, and other stakeholders to connect clients to necessary professional and personal supports. Develop and implement individualized transition plans and monitor progress toward successful community reintegration. Maintain accurate and timely documentation of client interactions, service plans, and outcomes. Provide outreach and follow‑up to reduce future admissions and promote client independence. Report directly to the Care Transitions Program Supervisor. Qualifications Knowledge of the characteristics and symptoms of Serious and Long‑Term Mental Illness (SDMI) in adults. Strong analytical and critical thinking skills. Skill in managing timelines and the logistics of complex cases. Ability to interact with diverse groups, providers, staff, and subject matter experts. Bachelor’s degree in a related human service field. Two years of experience in health, behavioral health, human services, or education. Other combinations of education and experience will be considered on a case‑by‑case basis. Preferred Qualifications Master’s degree in social work, public health, public administration, or a related field. Experience working with populations with SDMI and/or intellectual/developmental disabilities. Telework Eligibility The position may be eligible for partial telework within the state of Montana.
