Licensed Practical Nurse (LPN) Job Responsibilities Admissions, Discharge and Transfers of residents Medication requisitions with pharmacy and physicians Medication administration Following infection control, emergencies, and other center policies Collaborating with resident’s interdisciplinary team to provide the best outcomes Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy Work under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment. Deliver nursing care to patients/residents requiring long-term or rehabilitative care. Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident. According to state-specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response. In accordance with state-specific regulations, direct and supervise caregiving by other nursing personnel in selected situations. Maintain knowledge of necessary documentation requirements. Maintain knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc. Maintain confidentiality and patient/resident rights, regarding all patient/resident/personnel information.
