Weekend OASIS Registered Nurse

Hartford Healthcare seeks a Weekend OASIS Registered Nurse to provide comprehensive in-home assessments and skilled nursing care to adult and geriatric patients across our Connecticut service area. In this visiting nurse role, you will complete accurate, timely OASIS assessments, develop individualized care plans, administer treatments, and educate patients and families on disease management and medications. You’ll collaborate with an interdisciplinary team, leverage advanced technology for documentation, and practice within a culture that values compassion, teamwork, and ongoing professional growth.
