OASIS Registered Nurse

Hartford Healthcare seeks an OASIS Registered Nurse – HomeCare to deliver skilled nursing services to patients in their homes. The RN completes comprehensive OASIS assessments, develops and updates individualized care plans, administers medications and treatments, and provides wound care and chronic disease management. You will educate patients and families, coordinate with an interdisciplinary team, and document accurately in the EHR.
