Home Health Physical Therapist

This is a 13-week contract position.

AM shifts, Monday through Friday, with a guaranteed 40-hour work week.

We are seeking a dedicated Home Health Physical Therapist to provide high-quality care for patients in Southern Brazoria County. The ideal candidate will assist patients in regaining mobility and achieving maximum independence through personalized therapy interventions. You may also be asked to support other territories within the facility's service area as needed, typically within a 60-mile radius.

Responsibilities

• Conduct evaluations and develop personalized treatment plans for patients.
• Perform routine home visits, adhering to a standard productivity range of 28-32 points per week.
• Complete necessary documentation in Homecare Homebase and OASIS formats.
• Participate in weekly case conferences and collaborate with interdisciplinary teams.
• Ensure compliance with all relevant healthcare regulations and standards.

Required Experience / Certifications / Licensure

• Experience in Home Health and familiarity with OASIS documentation is required.
• Licensure as a Physical Therapist is necessary.
• Experience with Homecare Homebase documentation is a must; candidates may be considered without it at a reduced rate for training during the first two weeks.
