Job Title: MTP Discharge Specialist/Juvenile Rehabilitation Specialist Location: Chehalis, WA – Flexible/Hybrid Closes: Tuesday, June 09, 2026 The Department of Children, Youth, and Families (DCYF) is accepting applications for a Medicaid Discharge Planning (MTP) Discharge Specialist/Juvenile Rehabilitation Specialist to support Green Hill School and agencywide MTP/Medicaid Discharge Planning by coordinating the planning, implementation, and oversight of MTP 2.0 reentry and Medicaid-funded medical and behavioral health discharge planning activities and the related Reentry Targeted Care Management. The Medicaid Transformation Project (MTP) 2.0 within JR strengthens reentry planning, improved access to care, and ensures continuity of treatment for youth transitioning back into their communities.
The Opportunity: You will serve as a subject matter expert on MTP requirements and Medicaid policy, coordinating interdisciplinary discharge planning processes. This work supports least-restrictive, community-based transitions and continuity of care across behavioral health, medical, vocational, and social service systems. You will provide strategic leadership by integrating MTP requirements into JR policies, procedures, and operational practices and supports continuous quality improvement at the local and statewide levels. The role also serves as a resource with technical expertise to educate and engage young people in healthcare services and collaborate with community providers to promote successful reentry outcomes.
Some of what you'll do:
- Develop and oversee tracking systems for determining eligibility, release dates, and established levels of care for Medicaid Discharge Planning services.
- Serve as campus lead on healthcare coverage data and reporting.
- Identify trends, to include service gaps, opportunities for efficiency, achievements for acknowledgement, and outcome reporting.
- Coordinate the Medicaid screening process.
- Ensure consistent follow-up through communication and coordination between RTCM care planners and assigned JR Counselors, Clinicians, and regional Specialists.
- Serve as a liaison with DCYF partners and external stakeholders to support coordinated care and successful transitions.
- Provide ongoing communication and tracking to ensure expedited processes and services for young people with short-term stays.
- Develop training materials and marketing/informational resources for the campus related to Medicaid Discharge Planning.
- Educate young people regarding individual rights and available resources that are available through the new MTP program.
- Serve as a lead in developing local policies, procedures, and practices for achieving and/or maintaining compliance with facility licensing, professional credentialing, federal privacy protections (HIPAA/ 42 CFR Part 2), and accreditation with the National Commission on Correctional Healthcare (NCCHC).
- Partner with Compliance Administrator, facility Mental Health Specialist, and campus leadership to ensure alignment with health care discharge planning and agency goal for additional NCCHC accreditation for mental health services.
- Serve as a technical lead on developing capacities for submitting service encounters as required, and for eventual submission of billing to generate additional revenue streams through Medicaid reimbursement.
- Participate in local, statewide, and national oversight committees for program development in MTP.
Required Qualifications:
Equivalent combination of education and experience totaling six years. OR One year as a Juvenile Rehabilitation Residential Counselor (JRRC), Juvenile Rehabilitation Community Counselor (JRCC) or Juvenile Rehabilitation Counselor 2 (JRC2). OR A Bachelor's degree from an accredited institution AND two years of professional experience in casework, counseling, probation and parole, social services, planning, directing, and/or facilitation youth group activities or experience in a related field. AND The ability to take action to learn and grow. The ability to take action to meet the needs of others.
Note: A Master's degree in Psychology, Sociology, Social Work, Social Sciences, or in a related field will substitute for one year of experience.
In addition to those required qualifications, our ideal applicant will also have some or all of the following:
- Treatment and intervention principles and practices.
- Ability to relate to clients from diverse backgrounds and cultural groups.
- Knowledge of group behavior and dynamics, societal trends and influences related to juvenile delinquency.
- Adolescent development and juvenile rehabilitation practices and principles.
- Providing cognitive/behavior therapeutic interventions.
- Basic juvenile correctional system, rehabilitation objectives in juvenile institutions, basic interviewing and counseling theories and techniques, basic concepts, fundamentals and principles of individual and group behaviors, introductory psychology of deviant behavior and crisis intervention methods and techniques.
- Supervision and Lead techniques.
- Report writing.
- Ability to obtain pertinent information during interviews, assist in rehabilitation of juvenile offenders, implement counseling techniques, apply rules, regulations and procedures, appraise situations and remain calm and in control in crisis situations and de-escalation situations.
- Ability to speak and write clearly, establish and maintain effective working relationships with wide range and level of staff and others and establish rapport and gain confidence of youth.
