Resident Care Coordinator – Senior Living Chubbuck, ID About Savoy Life Savoy Life is a technology‑enabled healthcare services startup focused on making healthcare work for seniors and their caregivers. We aim to give seniors more quality time at home by coordinating care in homes and senior living communities. Position Overview The Resident Care Coordinator (RCC) will provide clinical and operational support to seniors across Chubbuck & Pocatello, Idaho. The role partners closely with Savoy Life RNs and providers to deliver care in senior living communities, individual homes, and other settings. Key Responsibilities Schedule and coordinate patient visits in the EHR to maximize RN and APP time. Answer inbound calls, texts, voicemails, and manage faxes. Enter and maintain patient data, scan, and print files for accurate record keeping. Identify and manage health plan insurance matters for residents. Support RN and APP by coordinating care to improve outcomes. Collaborate with local team and community resources to further patient care goals. Facilitate telehealth visits via tablet and conduct RCC‑led resident visits. Manage communications and maintain positive relationships with patients, caregivers, families, primary care providers, specialists, pharmacies, and senior living staff. Engage eligible residents, POAs, and families, educating them on Savoy services and securing consent. Qualifications High School Diploma or GED (or higher). At least 2 years in healthcare focused on care coordination or administrative support. Certification as Medical Assistant, Patient Care Technician, or equivalent preferred; otherwise 5+ years of healthcare experience. Experience in geriatric or senior living settings highly desirable. Valid driver’s license and comfortable driving between locations. Strong EMR documentation skills and comfort with technology. Ability to multitask and switch between resident visits and behind‑screen coordination. Positive, purpose‑driven attitude and adaptability to a dynamic startup environment.
